Right Atrial Approach for Ventricular Septal Perforation

چکیده
症例は84歳男性，2カ月前より全身倦怠感，数日前より安静時呼吸困難となり近医を受診した．心臓超音波検査で心室中隔基部に1.5 cmの欠損孔と左右シャント血流を認めた．亜急性期心筋梗塞による心室中隔穿孔の診断で緊急手術を施行した．右房斜切開すると三尖弁中隔尖の直下に穿孔部位を認めた．ウシ心のう膜パッチを用いたsandwich patch法で穿孔部位を閉鎖した．術後は遺残シャントなく，順調に経過し，第18病日にリハビリ目的で転院となった．
